II.1.5) Short Description of the contract or purchase(s) The London Borough of Barnet's department for Adult Social Services is seeking suitably qualified and experienced organisations to express an interest in being considered for participation in a tender process to provide an Intensive Supported Living Service. The service will be for up to fourteen people, across a number of sites, most of whom will be moving back to the local community following an extensive period living on an NHS Campus. The individuals concerned display high levels of challenge to services and have developed ritualistic and institutionalised behaviours over their many years in segregated services. They have a range of diagnoses, with most displaying Autism Spectrum Conditions, in addition to some physical and sensory impairments and mental health disabilities. The service users reflect the diverse background of residents in the borough and bidders will be required to demonstrate evidence of meeting diverse needs The successful bidder will also need to evidence a proven track record in proactively supporting people with Autism and Challenging Behaviours in a community setting. There would be an expectation that the provider will coordinate a structured and consistent holistic 24-hour support plan for each individual, including an active programme of day opportunities, in line with the Enablement focus adopted by Adult Social Services in Barnet. A key driver for learning disabilities services is the principles included in Valuing People Now. The contract will be awarded to one successful bidder initially for a period of three years with a possibility of extending it for a further two years following a service review. III.2) CONDITIONS FOR PARTICIPATION
III.2.1) Personal situation of economic operators, including requirements relating to enrolment on professional or trade registers
Any supplier may be disqualified who:

(a) is bankrupt or is being wound up, where his affairs are being administered by the court, where he has entered into an arrangement with creditors, where he has suspended business activities or is in any analogous situation arising from a similar procedure under national laws and regulations;
(b) is the subject of proceedings for a declaration of bankruptcy, for an order for compulsory winding up or administration by the court or of an arrangement with creditors or of any other similar proceedings under national laws and regulations;
(c) has been convicted by a judgment which has the force of res judicata in accordance with the legal provisions of the country of any offence concerning his professional conduct;
(d) has been guilty of grave professional misconduct proven by any means which the contracting authorities can demonstrate;
(e) has not fulfilled obligations relating to the payment of social security contributions in accordance with the legal provisions of the country in which he is established or with those of the country of the contracting authority;

III.2.3) Technical capacity
(a) a list of the works carried out over the past five years, accompanied by certificates of satisfactory execution for the most important works. These certificates shall indicate the value, date and site of the works and shall specify whether they were carried out according to the rules of the trade and properly completed. Where appropriate, the competent authority shall submit these certificates to the contracting authority direct;
(b) a list of the principal deliveries effected or the main services provided in the past three years, with the sums, dates and recipients, whether public or private, involved.
